Make a Referral

Referring a young person to SCI is straightforward

We accept referrals from professionals working with children and young people and will work with the referrer to understand their needs, circumstances and intended outcomes before intervention begins.

WHO CAN REFER?

Schools & Education

Schools, colleges, alternative provision and other education settings.

Youth Offending Teams

Professionals seeking targeted intervention for a young person they are supporting.

Children's Services

Social workers and other professionals supporting children and families

Early Help & Partner Services

Professionals working with young people who may benefit from targeted intervention.

SAFEGUARDING FROM THE START

Safeguarding is considered from the point of referral. Relevant information is reviewed before intervention begins to ensure that appropriate arrangements, risk assessments and safeguards are in place for the individual young person.

BEFORE MAKING A REFERRAL

It helps to have:
  • Basic details about the young person
  • Reason for referral
  • Current needs and concerns
  • Intended or desired outcomes
  • Relevant safeguarding information
  • Details of professionals currently involved

READY TO MAKE A REFERRAL?

Download and complete the SCI Referral Form, providing as much relevant information as possible to help us understand the young person's needs, circumstances and intended outcomes.
The form is best completed on a desktop or laptop.

Please download and save the form to your device before completing it. Once complete, save your changes and e mail the form to:

Please ensure that referral information is shared securely and only includes information relevant to the young person and the proposed intervention.
